Institute for Translational Population Health Research
The Institute for Translational Population Health Research is an independent nonprofit dedicated to advancing health, well-being, and social prosperity through evidence-based action.
United across public health, data science, behavioral health, and more.
Elmira, New York serves as our home and proving ground.
From forecasting to behavioral health, equity, and policy.
Every project co-designed with the communities we serve.
The Institute for Translational Population Health Research (ITPHR) is a nonprofit research, innovation, and community impact organization dedicated to advancing health, well-being, and social prosperity through evidence-based action.
Founded on the principle that research should improve lives, the Institute bridges the gap between scientific discovery and real-world implementation. We transform data into knowledge, knowledge into action, and action into measurable improvements in population health.
Our work integrates population health research, community engagement, health forecasting, behavioral health innovation, social determinants of health, and systems thinking to address some of the most pressing challenges facing communities today.
We believe that healthier communities are built not only through healthcare, but through stronger neighborhoods, economic opportunity, social connection, quality housing, educational attainment, and community resilience.
Mission. Vision.
A national leader in translational research.
Elmira, New York: our living laboratory.
The Institute proudly calls Elmira, New York its home and primary living laboratory.
Like many small and mid-sized American cities, Elmira faces complex challenges that influence health and quality of life. Population decline, housing deterioration, behavioral health concerns, substance use disorders, economic instability, vacant properties, and chronic disease burdens are interconnected issues that require innovative solutions.
Rather than viewing these challenges as barriers, we view them as opportunities for transformation.
Elmira provides a unique environment where researchers, healthcare providers, community organizations, local government, faith-based institutions, businesses, and residents can work together to develop, test, evaluate, and scale solutions that improve population health.
Our goal is to create evidence-based models in Elmira that can be replicated in communities across New York State and throughout the United States.
Every neighborhood, every community partnership, every intervention, and every outcome contributes to our understanding of how communities can become healthier, stronger, and more resilient.
Too often, valuable research stays disconnected from the communities that could benefit most.
The Institute was established to close that gap. We focus on translational research, which means moving scientific evidence beyond academic publications and into practical programs, policies, and interventions that improve lives.
Our interdisciplinary approach unites
Through these partnerships, we develop solutions that address the root causes of poor health while strengthening the systems that support community well-being.
The Social Prescription Program.
Prescribing connection, purpose, and community.
The Institute's signature initiative is the Social Prescription Program, an innovative community-based model designed to address the social and environmental conditions that influence health.
Many health challenges cannot be solved through medical treatment alone.
Individuals struggling with depression, anxiety, chronic disease, loneliness, housing insecurity, unemployment, food insecurity, or social isolation often require support that extends beyond the traditional healthcare system.
The Social Prescription Program connects individuals with community-based resources and opportunities that promote healing, resilience, and long-term well-being.
Population health forecasting and health intelligence.
The Institute is committed to helping communities prepare for the future.
Using advanced analytics, predictive modeling, trend analysis, and health intelligence systems, we identify emerging population health challenges before they become crises.
By understanding where communities are heading, leaders can make better decisions today.

They are partners in research.
We believe the people closest to community challenges are often closest to the solutions. That is why we prioritize community-engaged research and participatory approaches that elevate local voices and lived experiences.
Through collaboration, co-design, and shared decision-making, we ensure that our work remains grounded in the realities of the communities we serve.
Transforming evidence into impact.
The challenges facing communities today demand new thinking, stronger partnerships, and innovative solutions. From our home in Elmira, New York, we are building a model for how research, innovation, and community action come together to improve lives, strengthen communities, and create healthier futures.
