The Institute for Translational Population Health Research believes that every person deserves the opportunity to live a healthy, fulfilling, and meaningful life.

Health is not created solely in hospitals, clinics, or doctor's offices. Health is created in neighborhoods, schools, workplaces, parks, churches, community centers, homes, and social networks. It is shaped by the opportunities people have, the relationships they build, and the environments in which they live.

The Community Wellness Division exists to strengthen these foundations of health by developing programs, partnerships, and initiatives that improve well-being across the entire community.

One of the greatest public health challenges of our time is social isolation.

Research consistently shows that loneliness and social disconnection are associated with poorer physical health, behavioral health challenges, reduced quality of life, and increased healthcare utilization.

That is why many of our programs focus on strengthening community connections.

The Social Prescription Program is the cornerstone of our wellness strategy. Rather than focusing exclusively on medical interventions, social prescribing connects individuals with activities, services, and opportunities that improve social, emotional, and physical well-being.
