The Social Prescription Program is the Institute's flagship initiative and serves as the foundation of our approach to behavioral health and community wellness.
Many of the challenges affecting health are rooted not in medical conditions, but in social conditions. Loneliness, social isolation, unemployment, housing instability, lack of purpose, and limited community engagement can significantly impact physical and mental health.
The Social Prescription Program seeks to address these underlying factors by connecting individuals with opportunities that promote connection, belonging, and well-being.
Our goal: help individuals build stronger social connections while improving overall health outcomes.
